Reporting

Reporting is an essential element of grant making.  Final Reports are due one year after grant awarded or at the end of your project/program, whichever comes first. A report — final or interim — must be completed before another grant can be requested.

Cycle One Final Reports are due by August 30 a full year and two months following a grant being awarded or prior to applying for additional funding. (Ex. 2020 Cycle One paid out in June 2020 would be due August 30, 2021.)

Cycle Two Final Reports are due by February 28 a full year and two months following a grant being awarded or prior to applying for additional funding. (Ex. 2020 Cycle Two paid out in December 2020 would be due February 28, 2022.)


IMPORTANT NOTICE

Beginning with the 2022 Cycle One, the CHMF application process will change as follows:
Letter of Inquiry will only be accepted during a two-week period. 

Cycle One:  January 2-January 16 by 11:59 pm ET
Cycle Two:  July 1-July 15 by 11:59 ET 

Grant applications in each cycle for consideration by the CHMF Board will be limited to the first 100 applications received that are correctly completed and have the proper documentation.
